This programme utilises the same accident sequence shown in Effective Health and Safety Management, to look at why and how accidents are investigated. The objectives of the programme are; to identify all the causes of accidents and near misses especially the underlying causes, to come up with recommendations and to communicate and carry out a clear action plan to prevent them happening again.

The programme answers questions including:

  • Do all accidents need to be investigated?
  • What about near misses, should they be investigated too?
  • How do you go about an accident investigation?
  • What information do you need to collect?
  • Who needs to be involved?
  • What do you do once the causes of the accident have been uncovered?
